Community Reintegration Services
Supported Living/Semi-Independent Living Program
In this program, person are provided with supportive services by our professional and paraprofessional staff in their own homes or in apartment and home settings that are developed to meet their needs.
Physical Conditioning Program
Persons are provided with interventions from our staff within their homes or within a variety of locations and facilities in their area to develop and maintain routines of aerobic and muscle-strengthening activities as may be appropriate.
Social Recreational Program
Person are provided with activities in diverse community locations to promote the development and maintenance of participation in activities which they may find meaningful and reinforcing.
Academic Program
Our staff interface with schools, families and others involved in a student’s care and rehabilitation to develop plans to enhance academic performance by assessing needs for accommodations, modifications, special services, tutoring, cognitive rehabilitation and other supports that may benefit their progress.
Vocational Program
In this program, staff work with patients and those in existing business and industry to develop work try-outs, training, work-hardening, job-coaching, and supported and competitive employment placement.
Specialized Behavioral Programs
Our staff have extensive experience in developing, implementing and maintaining programs to modify and stabilize difficult behavioral issues that are oftentimes associated with neurological, emotional and physical conditions.
Integrated Programs
Behavior modifications, standby-assist, fading, cognitive rehabilitation and other professional and community supports are often integrated into combinations of the above programs to meet comprehensive care needs.
